What to Do With Kotaro in Sekiro Shadows Die Twice
One of the joys of FromSoftware games is the diverse cast of NPCs you will meet along your journey. One NPC you will meet in Sekiro Shadows Die Twice is Kotaro, a large, sad monk near Senpou Temple. So what exactly is Kotaro’s story? Let me show you what to do with Kotaro in Sekiro Shadows Die Twice.
Where is Kotaro’s Location?
Kotaro is located just outside of the Senpou Temple on Mt. Kongo against a tree. This tree is just outside the Senpou Temple along the path you need to take to make your way deeper into this area. When you speak with Kotaro he will mention that he wants a white flower. You will need to find one and bring it to him.
White and Red and White Flowers Locations
There are actually two flowers (pinwheels) you can collect and give to Kotaro, the white flower and the red and white flower. Both flowers are located in the Senpou Temple, Mt. Kongo area. For a better look at each location, check out these videos:

Depending on what outcome you want with Kotaro will determine which pinwheel to give him. For the best outcome (what he wants to do), give him the White Pinwheel.
What Happens When you Give Kotaro the White Pinwheel
Once you’ve collected the White Pinwheel, return to Kotaro and speak with him. Give him the White Pinwheel. Once you’ve given him the Pinwheel, he will ask you to send him away to be with his friends. To do this, use your “Divine Abduction” prosthetic on him. This causes him to disappear. Kotaro will reappear in the Halls of Illusion. Speak with him there to get the Taro Persimmon.
What Happens When you Give Kotaro the Red and White Pinwheel
If you opt to give Kotaro the Red and White Pinwheel instead, there are two options open to you. These two options involve other NPCs. Here’s what you can do.
Send Kotaro to the Surgeons
If you are really feeling evil you can send Kotaro to the Surgeons located in dungeon. Here Kotaro will become an experiment. Once the Surgeon questline is finished, Kotaro appears on the small island between the dungeon and the Senpou Temple. If you swim over to him, you will see he has red eyes.
Send Kotaro to Anayama the Peddler
Located just before the Outskirts Wall – Stairway Sculptor’s Idol is a merchant by the name of Anayama the Peddler. Anayama wants to learn about what the samurai need (in order to sell it to them). You need to eavesdrop on some soldiers on the bridge between the Old Grave and Snake Shride Sculptor Idols. Once you’ve eavesdropped, you will learn they need rice. Return to Anayama with this information. After doing this Anayama will mention that he needs someone big, strong and dumb to help him out. Send Kotaro his way.
